SOGLASJE UDELEŽENCA

 

Spodaj podpisani/-a _________________________ soglašam, da lahko Društvo športnih psov Slovenije, Puštal 12, Škofja Loka (v nadaljevanju DŠPS) kot upravljavec zbirke osebnih podatkov v skladu z določili Splošne uredbe o varstvu podatkov (GDPR), zbrane osebne podatke hrani, obdeluje in uporabljala zgolj za namen organizacije in izvedbe tekme v pasji vleki - 10. memorial Henrika Sečnika, kar izrecno potrjujem s spodnjim podpisom predmetne izjave. 

 

S podpisom tudi potrjujem, da sem seznanjen/-a in soglašam z naslednjimi informacijami v zvezi s hrambo in obdelavo osebnih podatkov:

 

DŠPS zbranih osebnih podatkov ne bo posredovalo drugim družbam ali tretjim osebam.

 

V skladu s to privolitvijo se bodo osebni podatki hranili trajno. Soglasje se lahko kadarkoli prekliče na podlagi zahtevka za preklic soglasja za hrambo in obdelavo zbranih osebnih podatkov, ki se pošlje po pošti na naslov DŠPS ali na elektronski naslov društva: drustvo.sportnih.psov@gmail.com.

 

Posameznik lahko od upravljavca v zvezi z obdelavo zbranih osebnih podatkov kadarkoli zahteva dostop do osebnih podatkov in popravek ali izbris osebnih podatkov ali omejitev obdelave osebnih podatkov. Posameznik je upravičen do ugovora obdelave osebnih podatkov ter do prenosljivosti osebnih podatkov. V kolikor posameznik meni, da upravljavec njegovih osebnih podatkov ne obdeluje v skladu s podanim soglasjem, lahko vloži pritožbo pri Informacijskemu pooblaščencu.

 

Spodaj podpisani udeleženec/udeleženka 10. memoriala Henrika Sečnika dovoljujem, da DŠPS obdeluje osebne podatke, zbrane s prijavo na tekmovanje v svojih zbirkah osebnih podatkov in izjavljam, da sem seznanjen, da lahko od DŠPS kadar koli zahtevam prenehanje uporabe osebnih podatkov za namene informiranja.

The undersigned ___________________ I agree that the Drustvo sportnih psov Slovenija, Puštal 12, Škofja Loka (hereinafter referred to as the DŠPS) as a personal data controller in accordance with the provisions of the GDPR, the collected personal data should be kept, processed and used solely for the purpose of the organization and the execution of the sleddog competition -10. Memorial Henrika Sečnika, which I explicitly certify with the following signature of the subject statement.

 

I also certify that I am aware of and agree with the following information regarding the storage and processing of personal data:

 

DŠPS will not provide the personal data collected to other companies or third parties.

 

In accordance with this consent, personal data will be retained permanently. Consent may be revoked at any time on the basis of a request for revocation of the consent to the retention and processing of the collected personal data sent by mail to the DŠPS address or to the e-mail address of the association: drustvo.sportnih.psov@gmail.com.

 

An individual may, at any time, require the controller to process the collected personal data from accessing personal data and rectification or erasure of personal data or restrictions on the processing of personal data. The individual is entitled to object to the processing of personal data and to the transfer ability of personal data. In so far as the individual considers that the controller does not process his personal data in accordance with the consent given, he may lodge a complaint with the Information Commissioner.

 

Undersigned Participant/participant 10. Memorial Henrika Urenika I allow Dšps to process personal data collected by applying for a competition in its personal data repositories and I declare that I am aware that, at any time, Dšps may request the cessation of the use of personal data for information purposes.
